Support Co-ordinator

North Wiltshire (Ref 32)

To support young people aged 16 -25 in Alabaré’s North Wiltshire housing projects toward independent living.

To lead, supervise and co-ordinate the work of the North Wiltshire team to ensure compliance to the Supporting People Contract.

To act as the single point of contact/referral for the Service, co-ordinating allocations and Support Plan reviews and providing a focal point for the team to facilitate a co-ordinated approach.

To track and monitor the support pathways of service users and hold multi-agency case review meetings where appropriate.

To carry caseloads, directly line manage and provide caseload supervision to key staff.

Grade 7, 37.5 hrs per week. Salary £18, 217 - £20,640 per annum

Requirements:

An understanding of the needs of homeless people, and knowledge of relevant legislation in particular Supporting People.   A professional, enthusiastic approach and ability to work within professional boundaries.

An appropriate qualification or the willingness to work towards one – i.e. NVQ Level 3 or equivalent.

You must hold a full current driving licence with access to a reliable vehicle.
